What is aromatization?
Aromatization is the biological process by which the aromatase enzyme converts testosterone into estradiol, a form of estrogen. In men on TRT, managing aromatization is important to prevent excessive estrogen levels.
Aromatization occurs when the aromatase enzyme breaks down testosterone and converts it into estradiol, a potent form of estrogen. This is a natural metabolic process in all men, but becomes clinically relevant during testosterone replacement therapy because the higher testosterone levels from TRT increase the substrate available for aromatization.
When aromatization proceeds unchecked during TRT, estradiol levels can rise to unwanted levels. Elevated estrogen in men on testosterone therapy is associated with problems including water retention, gynecomastia (breast tissue development), reduced libido, erectile dysfunction, mood changes, and increased cardiovascular risk. Different men aromatize at different rates depending on age, body composition, genetics, and liver health. Men with higher body fat tend to aromatize more because adipose tissue contains aromatase enzyme.
This is why monitoring estradiol levels and managing aromatization is a core part of proper TRT protocols. Clinics offering hormone therapy typically measure baseline and ongoing estradiol alongside testosterone, and some patients require aromatase inhibitors or dose adjustments to keep estrogen in a physiological range. Understanding aromatization helps men on therapy recognize why their provider checks these markers and what side effects might signal an imbalance.
