What is gonadorelin?

Gonadorelin is a gonadotropin-releasing hormone (GnRH) analog that stimulates the pituitary gland to maintain natural testosterone production and testicular function during TRT.

Gonadorelin is a synthetic version of GnRH (gonadotropin-releasing hormone) that signals the pituitary gland to release luteinizing hormone (LH) and follicle-stimulating hormone (FSH). These hormones then trigger the testes to produce testosterone and maintain sperm production. In testosterone replacement therapy, gonadorelin preserves testicular size and function that might otherwise atrophy from exogenous testosterone use.

The key distinction between gonadorelin and hCG (human chorionic gonadotropin) lies in their mechanisms. Gonadorelin works upstream by stimulating the pituitary itself, whereas hCG directly mimics LH and signals the testes. Both can maintain fertility and testicular volume during TRT, but they act at different points in the hormonal cascade. Some practitioners use gonadorelin when patients have pituitary sensitivity concerns or prefer a more physiologic restoration of the hypothalamic-pituitary-testicular axis. Others combine both agents or rotate between them to optimize results.

Gonadorelin addresses these goals by preventing the complete shutdown of endogenous testosterone production that can occur on TRT monotherapy alone.
