What is a selective estrogen receptor modulator (SERM)?

A selective estrogen receptor modulator (SERM) is a drug that blocks estrogen receptors in specific tissues, used in testosterone therapy to stimulate the body's own testosterone production rather than replacing it externally.

A SERM is a class of medication that selectively blocks or activates estrogen receptors depending on the tissue type. In the context of male hormone therapy, SERMs work by reducing estrogen's feedback on the hypothalamus and pituitary gland, which restarts the body's natural testosterone production pathway. Common examples used in this setting include clomiphene citrate and tamoxifen.

SERMs differ fundamentally from testosterone replacement therapy (TRT). With TRT, patients receive exogenous testosterone directly through injections, gels, or pellets. With SERMs, the goal is to preserve or restore the body's own testosterone-making capability by removing the hormonal brakes. This distinction matters because maintaining endogenous production may preserve testicular function and fertility in ways that direct TRT does not.

SERMs are typically prescribed during TRT protocols as supportive agents to prevent testicular atrophy or in cases where a patient seeks to discontinue external testosterone while maintaining hormonal levels. They may also be used during the restart phase after external testosterone therapy ends. Hormone therapy providers in Florida often evaluate whether SERM use fits a patient's individual needs and goals, since effectiveness and side effects vary.
